mine were via u/s and they measured the length of my closed cervix to make sure there was no funneling or shortening. my length was always good, but ask your ob what length they look for.

Basically, when you finish with the regular u/s, they use a wand attached to the same machine that they insert up inside you and it shows them the length of your cervix and if there is anything wrong with it ( shortening, funneling ). It is painless and i have been checked after every u/s since 18W. At 23W i still had a cervical measurement of 4, but they told me if i got down below 3 that they would put a cercelage in.

I had them in all my pregnancies. As mentioned it's to check your length and for funneling. I had a cerclage with my son at 22 weeks as I was 1cm dilated. With my twins I was 13 weeks but not dilated at all. I stayed over 5cm all my twin pregnancy.

It's uncomfortable to a degree (just like annuals are as well) if you never had one before, but it's really not painful at all. The put a condom and u/s gel on the top of the wand and then place it inside to check your cervix, it doesn't go in very far at all, since the baby is there and they want to check to make sure you are closed tight. With my kids could always get a really good view of the top of their head.
