I have the little trashbags, hand sanitizer, blankets, changing pad...I think that was it.

for clothes we did onesies for each and just a sleeper. Don't forget the bibs or burp clothes! Maybe a few small toys. Pacifiers if they use them.

    heres what I have/had:
    Bottles
    Formula
    Diapers (6-8)
    Wipes
    Changing Pad
Diposable waterproof pad to go on top of the pad
    baggies for dirty diapers
    Diaper creme
    Hand Sani
Onesies (2-4 depending on how long we would be gone)
2 Snap up pajama outfits (or more if gone for a long time)
Binkies (2-4 - cause we tend to loose them!)
    burp cloths (4)
    maybe a couple small toys/rattles
maybe a nasal aspirator (chloe hates boogies in her nose!)

Mostly all of the above.. But I also keep a bottle of water in case I run out of bottles. (I pack a bunch, but sometimes we are out longer than expected.) Also nail clippers, so if they are asleep in their seats and I am the passanger I can crawl back and clip them.

one onsie or footy pjs each
    nose sucker
    sanitizer
    brush
    extra binkie
Packets of formula or a small can

all of the above, but i would also lug a small thermos with hot water to warm up their bottles--oh and a nail clipper since i always realize their nails are too long when we are out!!

It changes as they age but the the beginning we had:
    2-burp clothes
    2- onsises
    2-recieving blankets
    2-bottles w/formula
nursing apron (we bf and ff for convience when out)
    4-6 diapers
    wipes
    2-binkies

YOu have so much to carry when double, I tried to keep it as light as possible.

In addition to what others have mentioned, I bring one of those Gerber receiving blankets - the ones that are worthless as a blanket because they are too small and thin - to put on top of the yucky public restroom changing tables.
