Vback with twins

    So I know I have already scheduled my c-section but I still can’t completely let go of the idea of a vaginal delivery. Up to this point the babies have stayed transverse but that doesn’t mean they wont move in the next 7 plus weeks. My doctor has made it quit clear that unless both babies go head down she would rather not try a vback. She also has said that because I have a previous c-section she is concerned about letting me go too long and that induction is not an option for me. I am just curious if anyone has had experience with this or what other doctors have said.
    I know ultimately it doesn’t matter how they get here as long as they are healthy but I never had a chance with my first and it looks like that is going to be the chase this time too and since this is most likely my last pregnancy I guess I am just struggling with excepting the fact that I will never have a vaginal delivery.

    I don't have any experiences with a vbac because Trevor was my c-section and he was my 3rd and last, but the hospital in our town won't do vbacs at all! The hospital 30 min. away does do them, however.

    I have had both kinds of deliveries, 2 vag. and 1 c-section, and honestly in the end the only thing that mattered was my healthy babies. I don't feel any closer to my 2 girls that were vaginal or anymore of Mom because of it.

    So, I think it's great you have a choice, many places don't let you choose vbac. I don't have any advice for you or experience, so I know I'm just rambling now, sorry! But I guess you have to decide what you think is best. If you truly want to experience a vaginal delivery and your OB is okay for it then I say go for it. Hopefully some other ladies can chime in to help you out. Good luck making the decision!!

    I would highly reccomend getting in contact with a local ICAN group, if one exists in your area ICAN website From there you can get in touch with other moms who have had successful VBAC's and may be able to find some that have had twin VBAC's. You'd also be able to get reccomendations for care providers that are VBAC supportive, as most claim they are but when it comes down to it - liability scares them into forcing a section on you.
    I know a local mom to me that actually had a twin VBAC homebirth Twin VBAC homebirth She is a super sweet mom and has been a great support for me, so feel free to email her - she is full of info!!!
    You need to do what you are most comfortable with - but please trust your body and your ability to birth these babies vaginally!!! You can do it - just make sure to surround yourself with those who support you and your desires.

    I know the fear of Vbacks has to do with uterine abruption..as in the pressure of laboring might cause the c-section scar to split. This with the fact that you are carrying twins would make that risk even greater. I'm no doctor, but I beleive this is what it boils down to, and uterine abruption is bad, and can even fatal for mom and baby.

    I would like to try a Vback should I have another child (twins were my first), but in your shoes, I personally would not.

    But, as the PP said, it is possible, and it's good that you should have the choice to choose that route, as long as you know the risks, etc.

    I honestly couldn't care about c-section or vaginal, as long as I get healthy babies...I don't think it makes you any less of a mommy!

    I do think c-sections are overdone in the U.S....but that's a whole other can of worms.

    Well, I'm going for vbac with this one...here are the stats my doc gave me...coming from the ACOG.

    there is a .5-1.2% chance of having a uterine rupture during spontaneous labor..
    there is a .7-1.5% chance of having a uterine rupture during a labor induced using pitocin.

    I have run across a few people trying vbac with twins. It's not an ideal situation...lots of docs are hesitant about it...but there are no stats saying that it's more risky than trying a singleton vbac. Obviously you have other risks you deal with during twin pg and labor which are separate from the vbac risks, but also must be considered.

    Personally, I'm not comfortable increasing my risks even slightly by allowing induction. Those stats were pitocin induction...many of the cervical preps used for induction make the risks even higher...so most docs will definitely not do them. And clearly from reading the birth stories on here lots of docs induce with twins just b/c it seems safer to get them out once you hit 38 weeks.

    I do have to clarify those stats i gave are for the risk of uterine rupture...this includes everything from a slight tear to the catastrophic ruptures most people think about. The risk of a catastrophic rupture is closer to 1% of those who have a uterine rupture...so that would be around 1% of 1%...so 1 out of 1000 vbacs. you actually have a much better chance of getting into a serious car accident than you do of having a uterine rupture that catastrophic..one in which your life is in danger, the baby's life is in danger and/or they might have to do a hysterectomy. by the way, this can actually happen in women even on their first child...many people don't realize that....and c/s actually carries a significant risk of needing a hysterectomy...most women don't realize that either...and most docs don't mention it. the risk is higher in vbac..but just wanted to point out that no delivery is perfectly free
    from these risks. C/s does come with other risks which vaginal delivery whether vbac or not does not have. Obviously and happily for all three "types" of delivery...vag, c/s and vbac..the risks are relatively small...which is why we don't hear about them too much.

    I encourage you to do some research on your own. Most of us twin moms become very risk adverse simply b/c of all the scary things we either hear or experience. It's a decision that only a well-informed person can make about themselves, not for anyone else. After doing the research I feel much more comfortable going for vbac than a repeat c/s... However, were I pg with twins again I'm not sure how i would feel.. I would need to do more research and talk to my doctors about it. I think your best bet would be to have the "ideal" situation going into vbac...meaning both twins vertex (so that a doctor doesn't need to put their hand up there messing around with your uterus in the middle of labor) and also with labor spontaneously starting on it's own. If these ducks were to all line up and you walked into L&D in active labor beyond 5cm, then I don't see why you couldn't try for vbac. A scar that holds up well enough to expand for twin development ought to be able to hold out during normal, natural contractions... I would try my hardest (and will be trying my hardest) to avoid addition of pitocin induced ctx... that's when I think you increase your odds slightly of UR. Your body will know what it can handle...but if someone else is pumping in too much stimulation that's when things get iffy....

    Hope you have a healthy and safe birth no matter what delivery you wind up with!!
