my 4yo is seeing a Kidney specialist -UPDATED!

    :( Okay 1st off this might get long so those of you who are just starting out just know that we need your prayers and good thoughts. Well I might as well start at the begining....

    RaeLynn started getting fevers before she was a year old, there was never any infection or anything so we always just treated her with tylenol and waited a few days for them to pass. I always knew there was SOMETHING going on but no idea what. At her 18 month appt we were seeing a new PA in town and I told him about the mystery fevers, he told me the next time she got one to bring her in right away and he would see what was up. Well 2 wks later... and low and behold she had a bladder infection. This is the 1st time I had ever heard about Hydronephrosis (bladder reflux). He was convinced that she was refluxing urine back into her kidneys and that she would outgrow it but sent us on to a Urologist. Well on a side not he mentioned that she had blood in her urine YIKES. He askes 1st if she has ever been molested WHAT???!!! Not the thing to ask a mother. Well she never has been, and the specialist we saw later was SO MAD he would even ask us that question and it had nothing to do with it.

    Well after some horrible tests and ultrasounds (if anyone's daughter has reflux you know what they go through) they concluded that she did NOT have reflux and sent us on our marry way. What about the blood in her urine? Why is she getting bladder infections? Noone ever answered those questions for me.

    The fevers continue...

    So on to her 3yo appt. this past summer and I tell yet again another new Pedi about the fevers. He thinks she is getting bladder infections because she gets constipated easily and puts her on all sorts of meds to loosen things up. And the fevers continue...

    Well fast forward to Wed (last week) and RaeLynn has had a fever for about 3 days, hovering about 99-100 degrees. She wakes up with a high of 104.5!!! I immediately called the Dr.s office and our regular pedi is booked so they send us to this young guy. Well when we get there RaeLynn has to pee so I grabbed a nurse and asked her to give me a cup for RaeLynn to pee in and she sent it straight to the lab before the Dr. even ordered it (it pays to be nice to the nurses!! LOL) Well it turned out she had tonsilitus (sp?) But then the urine tests come back... sure enough blood, protein and Kreotien (I didnt even know you could pee kreotien???) He is greatly alarmed. He says "its funny you came to me I just gave a speech about kids' urine and peeing blood" (he is actually going on to specialize in this field I think) So he tests me and her father (my ex... and he was an *** about it.. .a whole other story) for blood in our urine to see if it is a genetic urinary disease that she would have contracted from one of us. Well that wasnt it...

    So he calls me yesterday (the Dr. himself) to tell me that he has already called the Pediatric Kidney specialist in Billings (biggest hospital in the state) and set up an appointment. "and not to scare" me but she needs to be seen sooner rather than later, "within the next few days" YIKES AGAIN!!!! And that she will probaby require a kidney biopsy!!! And me... I didnt ask WHY she would need a biopsy, I am one of those who just DOES what the Drs say to do without asking enough questions. This guy told me "as a mother when you have a bad feeling you need to grab us drs by the head and bang it off a wall until we listen to you" This should have been caught 2 years ago, and now I am terrified that something is really really wrong that could have been fixed long ago.... I am so frustrated!!!

    Well that brings us to today... she is going to the specialist tomorrow (Wed.). I am scared/ nervous/ anxious and imagining the worst as only a mother can do! LOL Just pray for us please!!

    And if anyone has ever dealt with ANY sort of Kidney diseases or anything please let me know what could be going on.

    ETA: This new pedi also told me that in all her many urine tests over the last 2 years she has been peeing protien, something she SHOULD NOT be peeing and for some reason no one ever caught that or told me about it... I had no clue, I was only told about the blood in her urine. And when a child gets an ifection like strep throat, they can pee blood but her levels are WAY TO HIGH for that... just thought i would add this info.

    First of all, :hug99: . You've had a shock, but try not to beat yourself up over it. Although you had niggling doubts about those low grade fevers, your daughter was never acutely ill, so it is not that strange that you didn't bang any doctor's heads against any walls! Most of us would have done the same thing.

    Luckily, RaeLynn has not been acutely ill. That's good because it means that, while she may have something going on with her kidneys, the situation has not become dire. So, take a deep breath and let yourself off the hook here. You did look into it, twice. And only now have enough red flags gone off to warrant further investigation. Don't let unnecessary and unearned guilt make this more stressful than it already is! :hug99:

    One of my daughters had some kidney issues at birth and went through the tests you mentioned to determine if it was reflux. Reflux is a common cause of frequent UTIs in young girls. In boys it is rarer and more serious. Most reflux resolved on its own by the time the child is about age 5. Often a course of antibiotics is prescribed until the child has outgrown the reflux.

    My daughter's problem was not reflux, but an anomalic enlargement of one of her kidneys. This was essentially unremarkable and she was asymptomatic, but it required monitoring until the enlargement resolved on its own by about 1 year of age. In some cases, however, the enlarged space in a kidney does not resolve on its own, forms a pocket where urine can pool and lead to kidney infection. A persistent low grade fever might be an indicator of this, or more often there is a spell of acute illness which leads to the discovery and diagnosis.

    I don't know if this is what the ped urologist will be looking for in your daughter's case, but it may be one of the things. I suspect they plan to examine your daughter very thoroughly to get to the bottom of this, and you will finally know what is going on, and have a plan for treatment.

    As hard as this is to face, I think it is good that you will soon know what you are dealing with and know what the plan is from now on. If your daughter does have some type of kidney disease, you will be glad to know now and be able to start treating it ASAP.

    Here are some helpful links with overviews about kidney diseases. Hopefully, none of the serious ones need interest you, but it won't hurt to read it over so that you can be armed with a little knowledge and be better able to understand what the doctors are telling you as you and your daughter go through the testing and diagnosis stages at the hospital. It is OK to just do what the doctor says, usually, but when your child may have something a little more serious going on, it is even better to understand what your doctor says. Then you can deal with problems a little more effectively and also knowledge can help to ward off the anxiety that is inevitable when one of our children is ill!

    Thanks so much for your quick response. Yes we will be going on to Billings for a battery of tests... I think they will be covering everything kidney related, unless of course there is something obvious that standsout to the specialist. I know I am beating myself up and that doesnt help. Thanks for all your reassurance and kind words and the above links... I will let you know what what comes of tomorrows test.

    The other big stresser is our financial state. My Dh (raelynn's stepdad) had GREAT insurace via his Fed. government job which DOES cover RaeLynn. But still like many americans we are having trouble with credit debt and home loan debt and are debating on weather or not to secure unsecure debt. Dh worries about most of this and jokes " of course this couldnt have happend pre January 1st when our deductable was covered" LOL When it rains it pours! thanks for the info!!

    My 8 year old DD has always had bladder issues also. When she was younger they thought it was UTI's, or reflux but never really found the reason. She would get fevers and not be able to pass urine and they could not figure out why. She always had blood in her urine. About a year and a half ago the blood got to the point where you could see the drops of blood in the toilet. Her pediatrician sent us to a kidney specialist at a major childrens hosp (we did not get anywhere with the urologist). We went through a bunch of tests and found that she has too much calcium in her kidneys which causes irritation in her kidney filters and causes the blood in her kidneys and also the odd urine results that she was getting. The way to treat the issue is for her to drink a lot of water ( she should drink 64 oz a day!) and it is pretty much under control. The only times issues arise is when she gets sick and does not drink her water and then we have the same problem until she fully hydrates. ( she takes Pyridium when she can not urinate due to pain). The nephrologist initially told us at our consultation that since we went through so many tests prior to seeing him there was a good chance that it was not something major, and he was right. Going to him really solved her problems!!

    I understand how scared you feel and it is hard not knowing what is wrong. Just try to taKe it easy, there is nothing you can do at the moment and I always try not to stress or worry until a doctor tells me to!! Good luck with the specialist and let us know what happens!!!

    Wow this sounds ALOT like what we are dealing with. IT always gets worse when she is sick, thanks for the input, I am starting to look forward to the appt... just so I will finally have some answers.

    Thanks everyone. I read all the info in the above links and it actually calmed me down, but made my mother panic LOL! I feel armed with information and Dh is more level headed and will be with me this time to make sure we ask all the right questions. I will hopefully stop in tomorrow night and give an update!     My Dh has had nyphritis since he was 18 months old. He had protein,kreotin,and blood in his urine. But also he would stop peeing and his body would swell up. My dd had had uti since she was 4. These things can be very scary.Especially when its your own child. She has had all the tests(ultrasounds,catheter,xrays) and all showed normal. We still dont know what is wrong. She tends to "hold it". Which i think is the problem. So i know what you are going through. :hug99:

    First of all: This isn't your fault. You knew something wasn't right. You told the doctors numerous times. You finally found one that will listen :Clap:

    What you are describing of your dd reminds me a lot of what happened with my DBFs brother. Creotin, not feeling right, etc. He also had scary high blood pressure levels...are your dd's high? They never did find out what the cause of his kidney failure was. The family underwent the same tests you are, especially the parents, to see if it was hereditary. I guess I can tell you a best case outcome of a worst case scenario. DBFs brother needed a kidney transplant. DBF ended up being the donor. Both are doing well now.

    I hope it doesn't come to this for you dd, but know that they can do amazing things if it does. Ask away if you have any questions. I'm hoping things aren't that far gone with her and that they can figure out the problem and get her feeling better.

    My son has different issues as his main problems but he also has to get those nasty bladder and kidney tests every 6 months just to keep an eye on things. I feel so bad for him as the tests are painful and scary plus he cries out every time he pees for the next few days. I understand not wanting to put our kids through this unless we at least are getting valuable information back.

    Don't worry about things not being figured out sooner, none of us can go back in time to change things. It sounds like you did every thing right and the best anyone could have done at the time. You have now and the whole future to keep working on things to help your daughter. My brother had undiscovered kidney trouble his whole life, he made it in to his late twenties before any doctor suspected what was going on. At age 30 he got a kidney transplant, donated from my other brother. Now he is doing great and has a good life! I tell you this to show that it's never to late to get good medical treatment, and age 4 is still a young and lucky age for this kind of thing to be caught at!

    You sound like a great mom doing a great job! I sympathise that your family is going thru so much right now, I hope the worst is over.

    Okay so the appt was today. We didnt find out a whole lot, just basically preliminary blood work and the like. The Nephrologist we are seeing is not a pediatric specialist but she is passing all RaeLynn's info to a collegue at DUKE!! Which is scary yet encouraging. The two main suspects are Nephritus and Bottom Membrain Thinning Disease (which would be a thin membrane allowing protien and blood to pass into the urine). They are testing for what seems to be hundreds of things including the calcium in the kidneys thing, Lupus (the scary one for me to think about), Wills Tumor (highly unlikely), horseshoe kidney, nutcracker kidney and the list of very long and unledgable words written by the Dr. goes on!! LOL We will more than likely end up getting an ultra sound next week on her kidneys... and after all other options have been exhausted, a biopsy. The 1st two diseases may go away as she grows, but because she has been so symptamatic they may be more advanced (not the word I am looking for there??) anyway they are both correctable with surgery I believe. So we dont know how extensive the kidney damage is or isnt, but we are about 99% sure that it is Def. Kidney related now. It is good to have some answers still scary to think about the unknown.

    Thanks so so so much for all the responses and info, the Dr. was impressed I had looked all these things up last night and knew what she was talking about!! I should have at least half of the blood test results (and a promised phone call from the Dr herself) by Friday, the Lupus test and others should be in by next Monday. We will then know where we go from there. Thankd for all the support, it is so comforting to know I can come here and get such great help all the time!

    I'm glad things went ok yesterday. I had one question though. I really hope they didn't tell you that nepritis will go away as she grows or is correctable with surgery.Cause its not. Like i said my husband has had it since 18 months old, he is now 35 and still has it. It does however go into remission.He gets sick about every 5 years or so.Almost all the time he knows more about the illness then doctors,since he's had it his whole life.My prayers are with you. If you have any questions, please pm me.

    Thanks for the info. No I think the Membrane thinning disease can be corrected with surgery. With Nephritius she really didnt say HOW we would have to treat it but that it would have to be closely monitored as she grows. Does it cause damage to the kidneys? I think if she does have it they are going to biopsy to see the extent of the damage???? I am a little confused on all the different disseases and WHICH is being treated and HOW. Does that make sense?? I am not going to worry too much until I know a little more about the results. But is sound like we should be "hoping" for Nephritus... apparently the least severe disease on the list.

    I would appriciate all your info. on Nephritus that you can give me.

    ETA: But you are right I did say "both disease are correctable..." I was pretty mixed up on everything last night. But I do know it is the Nephrons in the kidneys that are not filtering correctly and that CANT be corrected. Thanks.

    I am so sorry that you and your daughter are going through this. I will keep her in my prayers My twin B Zoey was diagnosed with grade3-4 kidney reflex at 6 months old, she has gone through the horrible testing of VCUG's and ultrasounds numberous times. She acutally just went to the specialist last week and he told us good news. Her reflex has gotten better and is now down to a grade2. She takes sulfa everynight to help with any UTI or infections she might get. But I wanted to share her good news in hopes that good news is coming your way soon.

    Briana, if you go into your original post and click on edit, FULL EDIT, the window should come up giving you the original title box and you can edit it to say "Updated".

    Thanks so much for the update! I know it is all a lot to take in right now, but once the tests start coming back, the doctor will be able to begin ruling things out and narrowing the possiblities. At least then you will have a lot less to try to sort through.

    Good for you for keeping calm while you wait. I know how hard that can be!

    Okay the 1st round of tests are in. We still are not 100% sure what exactally she has BUT it is more than likely IGA Nephropathy, which was described to me as a type of Nephritis. The blood work was encouraging as is showed her kidney function is still pretty good and she didnt have more than a trace of protien in her urine, but of course she only has lots of protien when she is sick. This is all of course pending the outcome of the Lupus and AandA tests (testing for all autoimmune diseases) because the Dr is still thinking her extremely high fevers might be caused by something other than her kidneys.

    We are not sure what treatment we will be doing until our Dr. hears from her collegue at Duke and from the Denver Children's Hospital, and see what their recommendations are.

    From what I was told IGA Nephropathy is about the most 'mild' form of Nephritis you can have and she may even go on to outgrow her symptoms, but she also could get worse as time goes by. For now it sounds like for the next couple months her urine and blood will be tested every month and every time she has a fever or is ill. This will keep track of her kidney function and the protien. The 1st time she starts to spill protien the Dr. will order the kidney biopsy.

    For now that is all we know... hurry up and wait, is kind of how this process seems to be going! :D But at least this news is pretty encouraging and now we just have to wait on the other tests.

    I'm glad the specialist is following up so closely. When Kelli was going through her testing they also tested her for Lupus ( I have Lupus as well dx at 12) and it came back ok. Even is any of the automimmune tests come back positive it does not always mean Lupus ( unless they did Double stranded DNA testing). Alot of the bloodwork, ANA, IGA, SEd rates also go up with infection so make sure they are looking at everything. ANA results are also subjective and can change. It seems like you are in great hands and they will continue to keep a close eye on her. I'll keep you guys in my thoughts and prayers. Keep us posted!!

    I think I can almost feel some of the anxiety draining out of Montana! I'm so glad the field is narrowing and the outlook is brightening. Nothing worse than not knowing what's wrong, while also knowing it could be any number of serious things as well as less serious. It's just too much to take in. I am so glad that your doctor moved quickly and updated you as soon as she could.

    While I am sorry that RaeLynn has anything at all going on, I am relieved to read that it is looking more like one of the less severe possibilities.

    Sorry I didn't keep up better over the last week, but really I didn't have any new information. I guess I don't have alot of new information now.

    Okay the Dr. in Billings is still thinking that RaeLynn may have IGA Nephropathy, a type of Nephritis. The blood work came in and her kidney function is still great. So that means they don't have to do the biopsy or anything else to treat her for this right now. YAY! She wanted to see RaeLynn every month for the next year or so to monitor her kidney function. Then the Dr from Duke University called her and he is pretty hesitant to diagnose anything right now. Bleeding into the urine is pretty common in little girls when they are sick, RaeLynn pees blood all the time, it is just worse when she is sick. He also says fevers are pretty common in children and they can get up to 10 high fevers a year. Well that is all pretty good news, but I just want to make sure we don't miss anything this time around. So the Dr in Billings is going to see us every six months now and test the urine and blood whenever RaeLynn gets a fever or is sick. We will be keeping pretty close track of it for now. I am just glad her kidney function is normal. Oh and of course the Lupus and ANA tests came back negative, another wonderful answer to prayer.

    Briana! This is wonderful news! How good that RaeLynn will not have to undergo any more uncomfortable tests at least for now. It sounds like it is entirely possible that RaeLynn is going to be just fine!

    Please keep us updated through the year, but it sounds to me like you can breathe a big sigh of releif and I am so happy for you!

    What a stupid doctor. She will not have to have a bio. She doesn't need one. Kids pee blood all the time espeically when they have an infection. I hope that doctors deesn't specialize in kidneys because he is way off. What an idiot to scare you the bad.

    First of all bio are very hard on the kidney itself and a very invasive thing for the child. Your little one just has bladder infections, they will give you meds daily for that. Also they will do blood work to see how the kidneys are working.

    I wish I would of seen this sooner. Please son't go back to that doctor because he has no idea. If doesn't need to tell you things that aren't true.

    By the way what kid of germ was in the bladder that caused infection?

    Hey there, well it isnt always bladder infections... whenever she has ANY infection ie strep ear nose... she will pee higher amounts of blood, but she pees a little blood all the time. We are having her urine tested anytime she gets sick.

    We are actually seeing a DIFFERENT kidney specialist, but here in MT there isnt a pediatric specialist, so she called her friend at Duke who is, and apparently this is what HE thought. I agree with you though, I think he was rather hasty as he only READ her case file, our other Dr is still following up pretty closly. But I still would love to hear all the info you have on the kidneys.

    I don't know if this will make you feel any better, but I was diagnosed wiht IgA Nephropathy at 21 yrs old after they found I has protein in urine during a physical. I am now 33 and have not had any problems, very healthy and active my whole life and only trace amounts of protein leakage. Even through 2 pregnancies and one being twins I still didn't spill that much. So hopefully this mild form of kidney nephritis will not effect much the quality of your dd life in any drastic way. :)

    Wow, I am glad I came back and read this thread. My one year old grand=daughter is awaiting an appt with a peidatric nephrologist. About a month ago she got really really sick with fever of 104.5. Hospital diagnosed UTI after a bunch of the so uncomfortable tests (blood draw, catheritization). Doc there said children under 5 generally do not get UTI's unless there is an underlying problem. She had to complete the course of antibiotics and see her reg ped to get a referral. From what I have read they probably suspect reflux. All of your info has been helpful.
