Kristi, help!

    I've been lucky never to get thrush, but (TMI) I have a horrible yeast infection right now. Are the -azole treatments safe to use while BFing? (I have a Terconazole rx left over from when I was pg and they thought I had a YI but I knew I didn't.) I tried to research it on Kellymom, but only found stuff about thrush.

    As read from Pg 781 in Hales Mothers Milk and medications

    Terconazole is an antifungal primarily used for vaginal candidiasis. Ir is similar to fluconazole and itraconazole. When administerd intravaginally, only a limited amount (5-16%) is absorbed systemically (mean peak plasma level=6ng/ml). It is well absorbed orally. Even at high doses the drug is not mutagenic, nor fetotoxic. At high doses, terconazole is known to enter breastmilk in rodents although no data are available on human milk. The milk levels are probably too small to be clinically relevant.
    Pregnancy Risk Category C
    Lactation Risk Category L#
    Adult Concerns: Vaginal burning, itching, flu like symptoms
    Pediatric Concerns: None reported du to minimal studies

    Adult dose 5g daily

    Holly, must be something in the NE water, I have one too (actually it was the mastitis antibiotics that did it for me).
    My doc prescribed me terconazole 80mg suppositories because they are safe for BF. The pharmacy had to special order them however because its not something that is normally prescribed. Its a 3day bedtime rx.

    I just read this and am freaking out a bit. I had a yeast infection two weeks ago and used the monistat 3 without even thinking. Was this okay??? I am usually so careful.

    I've had my share of yeast infections while pregnant and nursing. You cannot take that magic pill (Difulcan). I don't do well with creams, so I have asked recently. Whenever I get put on antibiotics I automatically get a yeast infection. What is recommended is to take a 5/7 day treatment of monostat or generic of meds. I have asked pharmacists and doctors.

    Well, it was my own fault, actually. I got sucked into the TS arcade and sat around in sweaty running shorts too long. Dumb! :blush:

    For the record, Diflucan is ok when BF. This is what we did for a severe Thrush problem. It was a mega dose for 1 week and then an additional week of a lower dose. Thrush is a more aggressive form of yeast than what is typical in the vagina.
    Also a bath in some baking soda is good to, it's a pH adjuster.

    I would strongly suggest to talk to your medical provider about this. I asked/questioned a doctor and the pharmacist when I had to go on anti-biotics and they had very strong opinions on use of this while breastfeeding. The doctor even looked it up in her little book.

    As for toxicity, fluconazole is the least toxic of the azole antifungals. It is now cleared for use in infants 6 months and older in the US...suggesting that its really quite safe. Milk levels are published in my book, they are moderate, but subclinical. The infant will not receive enough via milk to affect treatment.

    As to whether this therapy is safe, yes. As to whether Candida is the organism involved, we'll have to wait on this answer.

    YES, it is okay in bfing/pumping moms at doses that are under 200mg per day, more than that is more sketchy.

    Jackie, I would ask your doc what year that book was published as the this data has changed recently.
