I am pretty sure he is constipated :(

He is straining, hasnt pooped since sometime yesterday (early), he normally poops several times a day.

He is on ranitidine for reflux. He has a belly button hernia that seems to have gotten larger in the last couple of days, although I can still push it back through. He was on Similac Advance mixed with a small amount of EBM. I have been switching them over to the Costco formula so I have been mixing Similac, Costco and some EBM, slowly increasing the Costco. We are at half Costco now. I have stopped pumping so I will be cutting that out soon too.

There are so many things I am dont even know where to start to look. What can I do for immediate relief and then what should I look at changing if anything first. I dont want to change too many things as I will never really know what was causing it. I have a call into the ped to see what he suggests.

Since he is so young, I think you should just go with what your pedi suggests. I know that is very normal for babies to not poop every day. Does he seem like he is in pain?

My DS was having the same problems, although he is 6 months old. Our pedi told us to lubricate a rectal thermometer with vaseline and insert it into the rectum a little bit (I think about half an inch) and slowly move it in & out or around in slow circles. We tried it & after about 20 seconds, we had a poop explosion (just don't try this on the living room floor like I stupidly did!!). He wound up feeling much, much better, thank goodness.

Definitely try the thermometer, we did it once for Hannah and it worked too!

I think you could try a little prune juice in a dropper or just a little water may help-I am too chicken to do the thermometer trick:)
Poor baby! I hope he feels better soon!

We deal with this with Max from time to time. Keep in mind that prune juice can cause gas, which means more discomfort for him. We finally did the rectal thermometer and that worked. DH ran up to the drug store and bought one with a flexible tip. We used lots of petroleum jelly, inserted about 1/2", and repeated about 5 or 6 times. Within 5 minutes he had relief. You can also try a warm bath because the water helps release tight abdominal muscles. Good luck and I hope your little guy finds some relief soon.

My pedi recommended pear juice and not prune juice (or apple). Just mix equal parts juice with water. I too don't like to stick things in the rectal area!

Since our NICU days we have been giving the boys between 2 and 4 mls of prune juice a day in their bottle with the vitamins in it to mask the taste and they love it. Works very well to grease the tubes.

my baby girl had this problem, we would use half a suppository that the dr. recommended and it usually lubricated her and stimulated the muscles in the anus and she would poop a few min later.

i hated those vitamins, they were most of the reason why my little girl had constipation. the pediatrician nixed those things asap after he found out the NICU sent us home with them.

Our daughter had this problem for a while before we figured switched formulas. Our pedi told us to give her 2 ounces of prune juice mixed with 2 ounces of water. This turned out to be too much, as she had a poop explosion, so we just halved it, which worked well. No gas or discomfort. Worked within a couple hours of giving it to her.

We have a little bit of progress. He poo'd a little and seems to be in less pain. The pedi told us to try docusate sodium - anyone ever try this?
