Absent Diastolic Flow - Slow progress

    We were last week that one of the twins is suffering from Absent Diastolic Flow and is not progressing (in terms of weight gain) as planned. Currently the Dr's have said that they will monitor the situation and at very best we can hope for them to leave things as they are.

    The following provides some more detail -

    We are currently on the 34th week (completes this Friday)
    Twin A is at 3.75lbs
    Twin B is at 2.42lbs and may be loosing weight.

    We are trying to stay positive and are confident that the Dr's will get them through, However we are really concerned and can't help thinking the damage that lack of oxygen may have caused.

    We would love to hear from people that may have faced a similar situation as us.

    Do you have mo/di twins? (in other words, do they share a placenta)

    Absent end diastolic flow is common with TTTS. If you have one placenta, get evaluated for TTTS right away. If you have it, they may deliver the babies.

    My twins were mo/di with TTTS. My donor baby had absent end diastolic flow for several weeks (at about 18 weeks). My recipient baby also had some fluctuations too. It is very scary indeed, but I know other people with TTTS whose babies had absent end diastolic flow for quite a while and their babies are ok. I think if you end up with reverse diastolic flow, they will deliver right away. That is much more dangerous.

    You can try to help them - if you aren't already on bedrest, I would go on bedrest. Eat plenty of calories, drink plenty of water and boost your protein intake.

    My situation was TTTS based, but ultimately a placenta problem, which this may be as well. Doing these things, in theory, may help your placenta do the best it can!

    If I were you, I'd be pushing for steroids, and monitoring them often, maybe a couple times a week, and if things change, delivery. It sounds like you may have IUGR in one oof your babies. The bedrest, calories and protein will help that too.

    Push to be monitored often. There are lots of people that have IUGR babies - one of mine is technically IUGR too. She was small but healthy!

    First off, :hug99: to you.

    My Baby B wound up with reverse flow and I needed an emer. c section. I was before you, at 30 weeks and they are fine and 110% healthy now.

    Definitely ask about steroid shots!

    All, I just wanted to thank you for your support as well as the information that you've provided. Some answers to the questions that you have asked:

    The twins do not share a placenta, my wife has had the steroid for lung development and is in hospital on complete bed rest since last week. They carry out a CTG (every 4 hours) and so far the heart rate for both is excellent.

    The suggestion on protein is great and I will ensure that she has as many egg whites as possible!!!

    Thank you for sharing your experiences, it really gives us great hope at this traumatic time. We are first time parents currently in India (otherwise home is Australia) due to work and reading your stories gets us through as we have no family here.

    Please keep us posted on your wife and children's conditions. I am so glad they are monitoroing all of them so carefully! It must be so hard to be going through this while so far away from home.

    Just wanted to say CONGRATS! My babies had TTTS (I know you said yours don't) and I ended up in the hospital on bedrest. I know how difficult it is for you guys and i wanted to say hang in there and you will be in my prayers/thoughts. Keep us updated!

    Well I have an update. My wife gave birth yesterday to a little girl (3.80lbs) and boy (2.60lbs). The Dr's felt that the blood flow situation was worseing and leaving it any longer would be dangerous to both.
    Both are currently in NICU and are breathing without any assistance. They plan on introducing some feeding to the girl(Lara) this weekend and then the boy (Rohan) maybe 2 o 3 days later.
    So far so good, and I really want to thank all of you for your lovely messages and words of encouragement.

    I hope the babies come out of the NICU soon and hope your wife recovers soon as well.
