Incident report from preschool

    Your child visited the school clinic/office. Below is a copy of the report:

    Description
    Karina hit in head by toy, wielded by another student. approximate half inch size crescent shaped swelling above left eyebrow. child is oriented and alert, symmetrical smile, purposeful movements of all extremities. identifies colors and numbers.

    Treatment
    ice/ cold pack attempted to apply to area, but child uncooperative.

    Oh that is hilarious. She has a bump, refused an ice pack because she wanted to get back to playing, and is fine. And they gave you a medical report! That is awesome... can you imagine how many of those you'd have to fill out if you were cataloging every bump or scrape at home? (it sounds like a very thorough and good preschool!)

    Oh mine hate ice as well, unless of course there isn't actually an injury then they want ice.

    What always cracks me up is it would be accompanied by an incident report for the other one saying that they hit "another student" in the head with a toy and the actions taken. There was a biting phase where I wasn't asking if there was an incident report, but I was asking if there was one or two! At least with two they weren't terrorizing the other kids!

    If it had been one of mine, it would have been accompanied by an bad behavior report for the other that matched up. "Hit another student in the head with a toy, leaving approximate half inch size crescent shaped swelling above left eyebrow."

    This is totally what I was thinking -- I fully expected the next email to be a report saying Kevan had "wielded a toy" against another student. LOL! (But it wasn't!)

    Yeah, she has a teeny, tiny little bump -- I never would have noticed if they hadn't told me. Apparently she cried for all of a minute. And, she doesn't like ice because it's cold -- she even told them, "too cold."

    Heehee, I'm still laughing. I know we live in a hyper-litigious society, but come on!

    That is hysterical. "Child uncooperative" was where I laughed out loud. That and the word "wielded." :rotflmbo:

    If I had one of those for every bump, scrape, and bloody nose, I'd have a binder full by now. I had to train our teacher last year not to call me at work every time someone had an accident -- I said, "Unless you need me to do something about it, it can wait till pickup time."

    When my kids and their friends were in the biting phase, I got three kinds of (verbal) reports:
    1 - "Amy/Sarah bit another child."
    2 - "Another child bit Amy/Sarah."
    3 - "Amy/Sarah bit her sister."

    Only in case #3 were they allowed to tell me who the "other child" was. (Of course now the kids are old enough to tell me.)

    Oh Geez! :rolleyes: I feel for the person who has to fill out a report on every single little incident.

    I do have copies of incident reports similar to that.

    Nolan was playing with a toy when another child tried to take it. He hit/bit/whatever the other child, but the skin was not broken.

    then in Meghan's cubby...

    Another child was playing with a toy and Meghan tried to take it. The other child hit/bit/whatever Meghan. The skin was not broken.

    I saved those ones for the baby book. :rofl:

    I just KNEW, it was going to be two reports, one for each child. .One saying ______ hit a child with a toy. and one saying ______ was hit with a toy. But that was funny just the same. They are way more detailed than my kids daycare. (its the same format, but just not so detailed)

    I used to get those all the time. Of course, they couldnt tell us who the child was, but when you get two reports for the same thing, its kinda obvious.

    I've gotten the, DD bit a "friend" at school, etc. Then the counter, DS was bitten by a "friend" at school. Teachers admitted it was the same incident, but reports had to be anonymous! :) I wish I'd saved them, and may have, but they're buried in art work piles by now.

    Even when it's not injury to a sibling, these reports still refer to a "friend" as the recipient or transgressor. Cracks me up.

    I like your report, I also laughed at the uncooperative. :)

    I think it's kinda funny, but in a way I'd like to recieve a report like that if my kids was involved in an incident. I know it doesn't seem like much of an injury, but they are definately covering their basis. And my DH the Neurology nurse says "they did a child mini neuro exam because she was hit in the head". I think it's pretty cool actually that they gave that much info. eventhough it is kinda funny to read it like that. Very cool that they e-mail it to you. I always get freaked when I see the daycare's name/# pop up on my cell phone. They give a "courtesy call" if something happens. Last time, it was DS getting hit in the head with a kid size golf club requiring a trip to the ER and a glued up forehead.

    :rofl: Hoo boy! Can you tell we live in the land of lawsuits? I mean, dude, I teach martial arts to kids, and if something happens, I just grab the mom or dad and say, "Hey, Lucas bumped his toe today and I gave him an ice pack."

    Actually, this is how medical professionals have to write...I am a nurse, so I am used to that. (I guess that's why it seems to typical to me, lol) It's mostly just the 'rules' for proper documentation, even if it sounds funny it's to cover people's butts if something comes up!

    I agree with above on on the day care teacher side.

    Anything that leaves a mark needs an accident report. At least they get to type it. I have to write them by hand. With my older aged kids, all I have to do it say "I don't want to do the paperwork so stop" and they stop (they don't like Kendra when she is in paperwork mode)

    Some things we don't do (technically we should be doing them on paper cuts and hang nails) but then there are parents who we are extra careful to document everything that the child complains about. (long experience.....)
