Preterm Water breaking?

Hi, just curious....
I know with preterm labor/contractions, they put you on meds if you are weeks from your due date, in hopes of stopping labor. But what do they do if your water breaks? I have a history of preterm rupture of membranes, and really don't want to take terbutaline or procardia (the common preterm drugs) for more than a day or so. I would take them, if it meant I could get the steroid shots in for the babies lung development. But, I am hoping to make to at least 32 weeks without needing this info. Does anyone know what they do if your water breaks early?

I am pretty sure once your water breaks you have to deliver within 24 hours.

 

I don't know the exact protocol as I am not a doctor but I have heard of people's water breaking in the early 20 weeks and maybe even someone in the early 30's and they were put on very strict hospital bedrest and monitered daily for infections. And at the first sign of distress in the mother or baby(s) or of infection they deliver immediatly.

 

Thanks for your responses. I didn't think they could do much if the water goes..... seems like an important piece! I am being so cautious since I had this with my first child. Hoping it won't break till I am in the mid 30's week wise.

 

I think it depend if water gone at all or leaking. water is regenerate itself so If it is leaking you will be monitor and I think can be with babies inside for another 10 days
if the water break completly the baby need to be taken out in 24 hours. this is something I remember from my OB explanation. so everything depend on situation.

 

I had PROM (premature rupture of membranes) with my first born, a singleton, at 30 weeks. I was hospitalized and monitored and received both steriod injections and that was about all they could do but sit back and watch. I had him at 31 weeks. He was in NICU for 30 days and came home without any issues (except would latch the little bugger - I pumped for 6 months... ahh, but that's another story!).

I believe I was also on IV antibiotics since I was strep B pos. I would leak various amounts depending on how much I moved. Sometimes I'd get up to go to the bathroom and gush enough to need new socks! I still had plenty of amniotic fluid. It is made continuously.

Not sure how much things change for twins. Good luck and hope you do not have to deal with this at all. My twins were born with no PROM (they did put me on progesterone injections the last approx 1/2 of my pregnancy - that sucked but was worth it). I think these is some published info that the progesterone injections are known to decrease risk of prom, if you have a history of prom. This is only for singletons but my peri thought it was still worthwhile to do with the twins.

Every case of PROM is different. It is not true that they must deliver within 24 hours. Unfortunately PROM usually makes your contractions start, so you do end up on contraction medication to keep delivery at bay, particularly if you are quite premature. You should ask your peri more about what he/she specifically would do and make sure you are comfortable with it. GL!

 

My water broke on twin A at just over 27 weeks. I delivered boys at 29 weeks when my labor started up again (they had stopped it with mag drip) I have a good friend whose water broke on one twin at 17 weeks. She delivered both at 29 weeks.

I am in the same situation, pregnant with twins and my water broke on one twin at 17 weeks and am going to try and wait it out in hopes I can make it to viability. Can you share any more about your friends story?

 

ITA. Just because your water breaks does not mean they will deliver you within 24 hours. I think there are different factors that come into play when making that decision to imminently deliver or not. They will put you in the hospital on strict bedrest, probably give you PTL meds, and watch for any sign of infection. When I was in the hospital on bedrest one of the nurses told me stories of women that experienced PROM and held off on delivery for up to 12 weeks! :shok:

FWIW, as far as the PTL meds go I was on both Procardia, Terb and Mag for 4 weeks and my babies are now 21 months old and perfectly healthy. I know it's not ideal to have to take those medications but if it means you can carry your babies longer, it is so worth it! Having 28 weekers is definitely a long NICU road. Every day you can keep those babies inside, is 3 days they won't have to endure the NICU.

I don't have any story that I know 1 hand to share with you. Just wanted to give you (((((HUGS)))). The hospital where I have been for the last 8 weeks due to PTL had a lady with triplets that stayed in the hospital for 10w w/ one of the bags leaking fluid! She delivered at 32w! I wish you to make it even further than her and will pray thar you do.
To Mollyelizabeth, as PP stated, a water leak does not mean that you will be delivered within 24hrs. You should ask your OB/peri what his/her protocol is. This should bring you some peace of mind. I hope that it doesn't happen to you and that you have an uneventful pregnancy this time around.

 

I agree - ask your doctor what their protocol is. When I was inpatient there was a mom there that had her water break, and she was there for about 4 weeks before she delivered.

I was on Terbutaline and Procardia and one other drug for about 4 weeks before my twins were born. I believe it kept me pregnant until 32 weeks. They weren't fun to take, but necessary. I did have the steroid shots at 24 weeks, so that was a relief.

 

Baby A's water broke and 2 1/2 hours later we had babies! I was 34 weeks, and labor started shortly after the water breaking.

 

They will try to keep you pregnant as long as they can, if they can hold off infection. My water broke at 30w5d, and they were going to try to keep me pregnant for at least one more week, but baby A was trying to push her cord out with her foot so it became safer to deliver her. I would have been in the hospital that entire time. But then again, I had already spent 4 weeks on hospital bedrest and a total of 15 weeks on bedrest.

I did not go into full out labor when my water broke. If you do, I'm not sure that meds can stop that anyways.
